Some of the most serious health conditions – diabetes, obesity, hypertension and heart disease – can be prevented or managed through simple lifestyle changes and healthier habits. Albany County Seniors can get personalized nutrition counseling to help manage their health at no cost from the Department for Aging’s Registered Dietitian Nutritionist. National Council on Aging’s website offers tools and resources to help older adults, including medication management, flu prevention, fall prevention, and managing chronic disease.
National Institutes of Health offers tips on how to stay healthy, get good health care, and manage lifestyle changes as you age, such as: Eating Safely; Eating Well As You Get Older; Benefits of Exercise; How to Stay Active; Falls and Older Adults; Healthy Eyes; and Long-Term Care.
Senior Hope A non-intensive outpatient not-for-profit clinic. Founded in 2002, Senior Hope's mission is to provide high quality, science based, compassionate and comprehensive services to mature members of our community, 50 years and older, and their families.
Catholic Charities Senior and Caregiver Support Services Programs include transportation, home delivered meals (meals on wheels), community nutrition, long term care ombudsman, caregiver support (counseling, referral, and respite services), senior dining centers and health insurance information, counseling & assistance.
Preventive visits and yearly wellness exams are covered by Medicare. The visit includes a review of your medical and social history and counseling about preventive services, including certain screenings, shots, and referrals for other care, if needed. It also may include developing or updating a personalized prevention plan to prevent disease and disability, based on your current health and risk factors.
